The spinal disc between two or more vertebrae is removed and the adjacent vertebrae are “fused” by bone grafts and/or metal devices secured by screws. The fusion can be performed through the abdomen, a procedure known as an anterior lumbar interbody fusion, or through the back, called posterior fusion. Spinal fusion may result in some loss of flexibility in the spine and requires a long recovery period to allow the bone grafts to grow and fuse the vertebrae together. Spinal fusion has been associated with an acceleration of disc degeneration at adjacent levels of the spine.

If you want to relieve your backaches once and for all, check out what science confirms really works. It’s definitely not popping painkillers. “Strong opioids such as fentanyl and oxycodone have little effect on persistent low back pain, and might even make the pain worse when used over the long term,” says Dr. Underwood. “We know they carry a substantial risk of addiction or dependence, overdose, or even death.” Dr. Buchbinder says they may be used for a short time, but then should be stopped as soon as possible because the harm outweighs the benefits. Yet doctors continue to prescribe the drugs because “it’s much easier to provide pain medication for people with disabling low back pain than providing multi-component pain management programs that will help people self-manage their pain better in the long term,” Dr. Underwood says. If you’re already taking these drugs, talk to an expert who can help wean you off, and see a new doctor about different approaches to your pain.

Back pain can arise when any of these components malfunctions. The facet joints are loaded with nerves, which can become irritated by friction between the joints, causing pain. The joints themselves may be “jammed, ” so that certain motions create pain. Degenerative disease such as osteoarthritis can cause the discs to bulge, exerting pressure on nerves. The vertebrae themselves may become misaligned, causing bones to rub against one another.

Numerous powerlifters over the years have come back following ‘career-ending injuries’ to set all-time personal records. Donnie Thompson is the only man to total 3,000 lbs (1,265 lb squat, 950 lb bench, 785 lb deadlift). Many people don’t know this, but several years back Donnie suffered a horrendous back injury and herniated three discs. He could barely walk, but he got out of bed and rehabbed himself every day. Within three months he was back to heavy squatting and setting personal records. Got that? Setting personal records three months following an injury that herniated 3 discs!

Activity Holidays I would like also point out that practically all Sarno's patients state that their pain gets worse after certain activity or inactivity. That's psychological conditioning. For someone, the pain gets worse when they sit down. For others when they start certain physical activity. This is explained by the pain strategy and conditioning. You would actually need to read the book in order to properly understand the pain strategy and conditioning.

When you have back pain, the best thing to do is rest until the pain subsides, right? Not necessarily. Too much rest can worsen certain types of back pain and decrease muscle strength — and strengthening and stretching the muscles may actually reduce or eliminate many types of back pain. Instead, start with gentle stretches and experiment to see how you can get moving without pain. Try going out for a slow, easy walk, and pick up the pace when you can. Remember, it's best to discuss your current fitness routine and any changes to it with your doctor to avoid aggravating your condition.
Recommended Books Many researchers seem to believe that low back pain is a modern problem. For instance, Waddell writes, “Observations of natural history and epidemiology suggest that low-back pain should be a benign, self-limiting condition, that low back-disability as opposed to pain is a relatively recent Western epidemic ….” In 2008, Martin et al found that, “The estimated proportion of persons with back or neck problems who self-reported physical functioning limitations increased from 20.7%… to 24.7% … 1997 to 2005,” which certainly shows that it is a growing problem and therefore likely to be worse now than in the past. A Spanish study (Jiménez-Sánchez et al) showed that “serious” musculoskeletal complaints (including a great deal of back pain, presumably) increased significantly from 1993 to 2001. Finally, Harkness et al did a nice job in 2005 of comparing rates of musculoskeletal pain (including low back pain) 40 years apart in the northwest of England, and found a large increase. In his books, Sarno also strongly portrays low back pain as a modern problem — though he doesn’t defend it . It’s hard to say if back pain actually is a modern problem, or whether it just tends to be described as such. Remember that human beings have a strong tendency to sensationalize and dramatize! Harkness pointed out in her study that the appearance of an increase “could be partly explained by the ‘worried well’. The ‘worried well’ are those patients who are concerned about their health, and attend their GP to seek reassurance about their well-being.” This is a great example of how hard it is to really be sure of anything!
