That’s a huge topic, but here’s one simple example of an extremely common problem with back pain science: control groups that don’t control. Rather than comparing a treatment to a good, carefully selected placebo, most studies use a comparison to a treatment that is allegedly neutral, underwhelming, or placebo-ish. That makes the results hard to interpret: if each works about the same, it could mean that the treatments are equally effective … or equally ineffective! So much back pain science has this problem — or any one of a dozen other weak points — that you can effectively ignore at least 80% of all back pain research, because it’s so far from the last word on anything. Good science is essential to solving these problems, but really good studies are also difficult to design and rare. BACK TO TEXT (30) ~ p. 31 Neurology and homeostasis. While back pain is sometimes caused by injury or muscle strain, many times it is a result of a degenerative spine condition that has developed with age and repetitive motion. Cyclooxygenase II inhibitors Physician Directory Degenerative bone and joint conditions: As we age, the water and protein content of the body's cartilage changes. This change results in weaker, thinner, and more fragile cartilage. Because both the discs and the joints that stack the vertebrae (facet joints) are partly composed of cartilage, these areas are subject to wear and tear over time (degenerative changes). Degeneration of the disc is called spondylosis. Spondylosis can be noted on X-rays of the spine as a narrowing of the normal "disc space" between the vertebrae. It is the deterioration of the disc tissue that predisposes the disc to herniation and localized lumbar pain ("lumbago") in older patients. Degenerative arthritis (osteoarthritis) of the facet joints is also a cause of localized lumbar pain that can be detected with plain X-ray testing. There is nothing in his model that contradicts the possibility that descending modulation of nociception is involved in TMS. He wrote extensively about the role of placebo effects in pain (his clinical results, and my own results, are difficult to reconcile with a placebo effect due to their duration). He also acknowledges that the mechanistic details of TMS remain fuzzy, leaving the door open to a variety of possible mechanisms connecting emotions to pain, weakness, and muscle tension.
