Limited bed rest. Once the mainstay of treatment for back pain, bed rest has fallen out of favor. Doctors now know it's better to keep moving, so that your muscles don't become stiff. Bed rest can still be useful relief from low back pain, particularly if your pain is so severe that it hurts to sit or stand. But try to limit it to a few hours at a time and for no more than one or two days.

Yoga is a mind–body practice that has been found to reduce functional disability, pain, and the use of pain medication in people with back pain. One study found that even a short but intense 7-day yoga course which included breathing exercises (pranayamas), physical postures (asanas), meditation, and a teaching of the concepts of yoga was able to improve spinal flexibility and reduce pain-related disability in people suffering from chronic lower back pain.15

However, his experience is significant, particularly in light of all the other evidence that structural deformities do not cause low back pain. It’s not a great leap! With regards to spondylolisthesis, Sarno writes: “I have had a number of patients with this abnormality and have found in each case that the patient also had TMS [same as MPS — PI]. In every case the pain was eliminated by proper treatment for TMS, suggeting that spondylolisthesis was not the cause” (p25). This is followed by a “dramatic case history.” With regards to stenosis, “… it has been my experience this ‘abnormality’ is rarely responsible for leg or back pain” (p27). For my part, for now I accept that these two conditions may cause pain, but not inevitably — and I eagerly await the day when research is done to investigate Sarno’s claim.

Sarno coined a term for his diagnosis — “tension myoneural syndrome” — and believed many people could overcome their pain simply by acknowledging its psychosomatic origins. He instructed many of his patients to journal or seek out psychotherapy, to gain a better understanding of the emotions they were repressing that generated their chronic back pain. He also advised they immediately resume physical activity, that it would make them better and not worse.

Assumption 3 (implicit): ‘The Alexander Technique is somehow similar in its approach or efficacy to working with posture when doing yoga or using a standing desk’. As I hope I have argued by now, the Alexander Technique is radically different from yoga or changing one’s posture by standing at a desk rather than sitting. It does not attempt to address posture directly, by, for example, trying to ‘stand up straight’ or ‘pull one’s shoulders back’, etc. All such ‘posture’ approaches make the unwarranted assumption that it is possible to be shown what good posture is, and then achieve it by an exercise of will (‘doing something’). Alexander pointed out that that this belief is a delusion, since for the majority of people their kinaesthesia (sense of muscle tension and relation of body part to part) is unreliable, and cannot be trusted as a guide to better postural balance or co-ordination.
