This change results in weaker, thinner, and more fragile cartilage. Because both the discs and the joints that stack the vertebrae (facet joints) are partly composed of cartilage, these areas are subject to wear and tear over time (degenerative changes). Degeneration of the disc is called spondylosis. Spondylosis can be noted on X-rays of the spine as a narrowing of the normal "disc space" between the vertebrae. It is the deterioration of the disc tissue that predisposes the disc to herniation and localized lumbar pain ("lumbago") in older patients. Degenerative arthritis (osteoarthritis) of the facet joints is also a cause of localized lumbar pain that can be detected with plain X-ray testing. These causes of degenerative back pain are usually treated conservatively with intermittent heat, rest, rehabilitative exercises, and medications to relieve pain, muscle spasm, and inflammation. Preliminary research suggests that hypnotherapy may be of some use in the treatment of low back pain. For instance, a pilot study published in the International Journal of Clinical and Experimental Hypnosis found that a four-session hypnosis program (combined with a psychological education program) significantly reduced pain intensity and led to improvements in mood among patients with chronic low back pain. At one year, those with chronic low back pain usually continue to have moderate pain and disability.[2] People at higher risk of long-term disability include those with poor coping skills or with fear of activity (2.5 times more likely to have poor outcomes at one year),[93] those with a poor ability to cope with pain, functional impairments, poor general health, or a significant psychiatric or psychological component to the pain (Waddell's signs).[93] Corrections & Clarifications Yoga as a fitness modality has grown significantly in the west over ... I often spend at least one hour or more doing an Egoscue exercise called “The Tower.” It’s simple – you only need to lie on the floor and allow your pelvis and thoracic spine to relax. I found this exercise tremendously helpful for treating my chronic low back pain, which is now gone. Well said, but perhaps a bit wordy. Here’s the simple version: patients believe back pain is caused by structural fragility, and careers are built on catering to that belief. I would also say that it is difficult to alter that belief in anyone, patient or professional. This preoccupation with fragility isn’t just reinforced by the practices of many therapists, it’s a major reason for them. "If we can appreciate that pain is a protective device, not a measure of tissue damage, if we can communicate that to people, then we change the game," Professor Moseley said. The spinal disc between two or more vertebrae is removed and the adjacent vertebrae are “fused” by bone grafts and/or metal devices secured by screws. The fusion can be performed through the abdomen, a procedure known as an anterior lumbar interbody fusion, or through the back, called posterior fusion. Spinal fusion may result in some loss of flexibility in the spine and requires a long recovery period to allow the bone grafts to grow and fuse the vertebrae together. Spinal fusion has been associated with an acceleration of disc degeneration at adjacent levels of the spine. It's possible to make a moist heat pack with common household items. Given the prevalence of back pain, you would think we'd have treatment for it down pat. Not even close. "The challenge is that you can't see injuries to tendons, ligaments, and muscles the way you can bone fractures and herniated disks," says Jeffrey Katz, MD, a professor of medicine and orthopedic surgery at Brigham and Women's Hospital in Boston and the author of Heal Your Aching Back. Despite diagnostic advances, doctors can't pinpoint an exact cause for as many as 85 percent of back problems, which makes them tricky to treat. Spinal manipulation, for example, is controversial — some docs say it does more harm than good — but it's the only remedy that got me off the couch and back on my bike. "Chiropractic care is not without concerns; then again, neither is traditional medicine, particularly when you're dealing with a problem like back pain, which has no easy, one-size-fits-all fix," says Mark Moyad, MD, a FITNESS advisory board member and the director of preventive and alternative medicine at the University of Michigan Medical Center. The treatment of a herniated disc is based on pushing back the inner layer of the intervertebral disc back into place. When this happens the pressure on the nerve reduces and the intervertebral disc can recover. But when these muscles are reacting to slow or don’t have enough endurance, your large back muscles take over the task. They get exhausted very quickly.  This results in lower back pain after running. Begin an approved exercise program to support your back and abdomen.
