Opioid analgesics: These drugs are considered an option for pain control in acute back pain. The use of these medications is associated with serious side effects, including dependence, sedation, decreased reaction time, nausea, and clouded judgment. One of the most troublesome side effects is constipation. This occurs in a large percentage of people taking this type of medication for more than a few days. A few studies support their short-term use for temporary pain relief. Their use, however, does not speed recovery.
Sleep can be elusive during pregnancy, especially if your back is hurting, because most women tend to sleep on their sides. Olderman likens the spine in this position to a telephone wire that's hanging between the two poles of your shoulders and your pelvis. "The spine drops down to the bed just like a telephone wire would drop between two poles," he says. To support your back and prevent pain, fold a thick towel lengthwise and place it perpendicular to your body at the point on your waist where your spine is its most "droopy." The towel's length will ensure support without requiring readjustment, even if you roll over during the night. Or try sleeping on your side with a pillow between your knees to reduce the strain on your pelvis and lower back. Side sleeping is important as the pregnancy develops past the first trimester -- it optimizes blood flow to the baby, Dr. Rosser says. Sleeping flat on your back is not good for you or the baby; it is also very uncomfortable, she adds. Body pillows are helpful for supporting your chest, hips, and lower back. Experts advise sleeping on a firm mattress (Dr. Rosser suggests placing a wooden board under your mattress if you have a soft one), and following smart sleep habits such as maintaining a consistent sleep schedule and relaxing bedtime routine, using your bed only for sleep and sex, avoiding bright screens just before bed, keeping the space cool and dark, and not eating for at least two to three hours before bedtime.

Prevention and treatment of low back pain: evidence, challenges, and promising directions << Previous article Bony encroachment: Any condition that results in movement or growth of the vertebrae of the lumbar spine can limit the space (encroachment) for the adjacent spinal cord and nerves. Causes of bony encroachment of the spinal nerves include foraminal narrowing (narrowing of the portal through which the spinal nerve passes from the spinal column, out of the spinal canal to the body, commonly as a result of arthritis), spondylolisthesis (slippage of one vertebra relative to another), and spinal stenosis (compression of the nerve roots or spinal cord by bony spurs or other soft tissues in the spinal canal). Spinal-nerve compression in these conditions can lead to sciatica pain that radiates down the lower extremities. Spinal stenosis can cause lower-extremity pains that worsen with walking and are relieved by resting (mimicking the pains of poor circulation). Treatment of these afflictions varies, depending on their severity, and ranges from rest and exercises to epidural cortisone injections and surgical decompression by removing the bone that is compressing the nervous tissue.

Cobra Stretch Muscle relaxants: Muscle spasm is not universally accepted as a cause of back pain, and most relaxants have no effect on muscle spasm. Muscle relaxants may be more effective than a placebo (sugar pill) in treating back pain, but none has been shown to be superior to NSAIDs. No additional benefit is gained by using muscle relaxants in combination with NSAIDs over using NSAIDs alone. Muscle relaxants cause drowsiness in up to 30% of people taking them. Back Care Ergonomics “The Pain Perplex,” a chapter in the book Complications, by Atul Gawande. Gawande’s entire book is worth reading, but his chapter on pain physiology is certainly the best summary of the subject I have ever read, and a terrific reminder that good writing for a general audience can be just as illuminating for professionals. Anyone struggling with a pain problem should buy the book for this chapter alone, though you are likely to enjoy the whole thing. Much of the chapter focuses on one of the most interesting stories of low back pain I’ve read, and it is a responsible and rational account — although Gawande, like most doctors, seems to be unaware of the clinical significance, or even existence, of myofascial trigger points.
