Patients with shingles usually have had chickenpox earlier in life. The herpes virus that causes chickenpox is believed to exist in a dormant state within the spinal nerve roots long after the chickenpox resolves. In people with shingles, this virus reactivates to cause infection along the sensory nerve, leading to nerve pain and usually an outbreak of shingles (tiny blisters on the same side of the body and at the same nerve level). The back pain in patients with shingles of the lumbar area can precede the skin rash by days. Successive crops of tiny blisters can appear for several days and clear with crusty inflammation in one to two weeks. Patients occasionally are left with a more chronic nerve pain (postherpetic neuralgia). Treatment can involve symptomatic relief with lotions, such as calamine, or medications, such as acyclovir (Zovirax), for the infection and pregabalin (Lyrica) or lidocaine (Lidoderm) patches for the pain.

The last 2 months were the worst of my entire life... Could not move or get up in the morning without lying in awkward positions and just going through the pain before I limped and hobbled to work (which I had no choice but to do). I went through physical therapy, a chiropractor, multiple visits to the doctors office and a lot of medication. Sleepless nights, constant moving and re positioning, leaning to my right while walking and sitting (a very awkward side-lean), lying on the ground in pain so horrible I could not hold back the tears in front of my entire family...in the family room... I couldn't hold my baby or carry him or help with chores around the house... I had to be in bed and felt useless. I was literally looking for anything that would help, because everything I tried was just not working.

Bleeding in the pelvis is rare without significant trauma and is usually seen in patients who are taking blood-thinning medications, such as warfarin (Coumadin). In these patients, a rapid-onset sciatica pain can be a sign of bleeding in the back of the pelvis and abdomen that is compressing the spinal nerves as they exit to the lower extremities. Infection of the pelvis is infrequent but can be a complication of conditions such as diverticulosis, Crohn's disease, ulcerative colitis, pelvic inflammatory disease with infection of the Fallopian tubes or uterus, and even appendicitis. Pelvic infection is a serious complication of these conditions and is often associated with fever, lowering of blood pressure, and a life-threatening state.

I had a back injury as a result of repetitive lifting in a work environment. Three herniated disks, the upper one being the most serious as it is against a nerve root Before being allowed to see a Dr the HR department made me go first to a chiropractor. I got relief after the first visit. Maintained after the second and the last two made it worse. Finally was sent on to a DR. I took a lot of prednisone and had cortisone shots in the back. Time did more then the medication... medication caused other health problems. The exercise was the most helpful treatment but the hardest to start. The shooting pain wasn't just down the leg but around my body... pretty much anyplace from my waist to my knees. I found that cutting caffeine and really pushing water was when I had the first noticeable improvement. Allowing the body to rehydrate the disks was something I learned from a nurse practitioner and it allowed me to really begin the exercise therapy. Others recommended bicycling. I cannot do that still. It causes too much side to side movement on the lower back. The only bicycling I can do is on a recumbent stationary. My greatest luck has been with pilates type exercises for the core and light to moderate weight training. Even after five years I cannot stand at the counter and do dishes with out significant pain. I cannot stand on concrete floors for more then about forty-five minutes without developing a limp. For pain, my best friend is my TENS machine.
Epidural steroid injections are most commonly used in situations of radicular pain, which is a radiating pain that is transmitted away from the spine by an irritated spinal nerve.
